Continuity of Care
Following feedback from patients about difficulties seeing their usual GP, we are making changes to improve continuity of care
Microteams
- Dr Rebecca Fowler
- Dr James Latchford
- Dr Jessica Watson
- Dr Harriet Delap
- Dr Tasmin Patel
- Dr Sam Davies
- Dr Helen Pyke
- Dr Anita Vosper
- Dr Charlie Daniels
- Dr Nicola Lloyd-Powis
- Dr Andy Hamilton
- Dr Muntarin Kakita
We’re now aiming to book routine appointments with your registered GP wherever possible. This may sometimes mean a slightly longer wait, but it helps your GP get to know you and your health better over time.
If your registered GP has a longer backlog than usual, you may be offered a routine appointment with someone in their microteam – a small group of GPs who share care of a smaller patient group.
If you regularly see a different GP than the one you're registered with, please let us know so we can update your records.
Please note: we will only make this change once.
Urgent and specialist appointments remain unchanged. You will still see:
- Paramedics - for same day and urgent issues
- Pharmacists - for medication queries and reviews
- Physiotherapists - for muscle and joint (MSK) problems
- Mental health nurse - for mental health support
Why Continuity Matters:
- Less time repeating your history and a more satisfying experience
- More efficient care with fewer A&E visits and hospital admissions
- Stronger trust in your GP, improving your health outcomes and care quality
